Caretaker

The Caretaker role is not just about picking up the reins and steering a practice, it’s also about reviewing its present operational status so its sustainability and resilience can be rebuilt. We specialise in incorporating new models of care and technology into practices, allowing them to deliver care in a personalised more community-based way whilst weathering the current pressures on General Practice. Initial steps in our care taking program will involve reviews of each of the areas below.

Review of Governance Structures​

Review current structures of Governance, including: 

  • Service user, carer and public involvement
  • Information Governance - SARS
  • Risk management
  • Clinical audits
  • Significant event management 
  • Education and training
  • Clinical effectiveness
  • Safeguarding processes
  • Infection control 
  • Policies and procedures
  • CQC standards

Operations Process Mapping

This involves a review of present operational processes including clinical and administrative systems. This is particularly useful to allow alignment and configuration of processes that will drive efficiencies and savings for working at scale.




Quality Performance Review

A review is undertaken looking at the performance of the practice related to its contract and any enhanced services. The potential for improving QOF results and any additional income is explored and recommendations made. The processes related to claims is reviewed including the coding and prevalence of chronic health conditions

Practice Population and Appointment

Data Extraction & Analysis

Data is extracted from the clinical system the practice uses in an aggregate form. This includes data about:

• Basic Population Statistics

• Appointment usage across age/gender

• Appointment types

• The wait time for appointments

• Appointment usage and capacity across roles

• Use of appointments by frequent attenders


Staff and Patient Climate Survey

This online survey is designed to discover the current feelings and opinions of all staff and patients on relevant topics.

The survey is anonymous and there is a suite of questions which can be chosen from or bespoke questions developed to answer any specific concerns. The questions come in a format of either multiple choice answers or free written responses.

The answers to the survey are compiled into a report

and presentation. This can then be fed back and used to facilitate and direct change.

Succession Planning

Co-Formation will aim to steady a practice initially and then through transformation build resilience. Succession planning is important for the future practice’s sustainability and ideally this planning would commence at initial engagement. Knowing the plans for the surgery will allow any service redesign to fit in with the future ethos. The aim is always to enable a smooth handover with minimal disruption to staff and the running of the practice.
